Athletic Permission Form (internet form)
All students will travel together, as a team, to all sporting events in school owned or leased vehicles. Parents may take their children home from the event and in so doing, relieve St. Stephens Lutheran School (SSLS) of all liability. Anyone wishing their child to ride home with another adult, must indicate on this form with whom the student is to travel and thus relieve SSLS its liability and its responsibility.
The undersigned hereby requests and gives permission to participate on the athletic trip, and with this signed agreement absolves the teacher, coach, St. Stephens Lutheran School, and any and all members of its governing boards and any responsibility for safety, welfare, health and well-care of the child named below, beyond such matters as may be called reasonable care for children in the custody of a teacher/coach and subject to the teacher's/coach's clear instructions, and assumes personally and exclusively all responsibility and liability for accident, injury, etc., which may occur to the named child during the time of the specific activity as set forth at the beginning of the paragraph.
